    What to Bring for Your Short-Term Rehabilitation after Surgery.

    To help you prepare for your short-term rehabilitation stay, we've outlined a packing checklist for stay at Church Home.

    Preparing and organizing for pre- and post-surgery can be overwhelming. With many surgical procedures often short or long-term rehabilitation is required. If your doctor refers you to skilled short-term rehabilitation, Church Home LifeSpring can support your recovery needs.

    Church Home will aid you in your rehabilitation by allowing you to fully focus on your recovery process. We provide a safe, welcoming, and comprehensive rehabilitation facility.

    To help you prepare, we encourage our patients to pack key items that will simplify their stay as help you feel comfortable.  We’ve outlined a packing checklist for your short-term stay at Church Home.

    1. Important Paperwork Government ID
      • Health Care and Personal Identification. 
        • Social Security card
        • Medicare and Medicare Part D cards. 
        • Health insurance cards
        • Government issued identification e.g. Driver’s License, Military Veteran 
        • Discharge Papers

      • Advance Directives
        • Living Will
        • Financial Power of Attorney,
        • Health care proxy or Health Care Power of Attorney

      • Doctor's Order
        • A copy of your doctor's orders and hospital discharge papers

      • A list of any medicines
        • Current list of medication, dosage, and the time you take them.  

      • Emergency Contact List
        • List of family members and friends' phone numbers and addresses.
    1. Clothing and Personal items

    We encourage patients to pack a 1-week set of clothing that is comfortable and easy to get on and off. What to consider. Packing a suitecase

      • Shirts 
      • Elastic waist pants or shorts. 
      • Pajamas
      • Robe 
      • Socks and undergarment
      • Rubber sole slippers
      • Comfortable shoes for physical activities or therapy
    1. Assistive Devices

      • Hearing aids
      • CPAP Machines
      • Splints, braces, or orthotics
      • Dentures
    1. Personal Hygiene
      • Personal hygiene products e.g. soap, toothpaste, deodorant, etc.
      • Hairbrush or comb 
      • Haircare products

    What NOT to Bring

    We do not recommend you bring anything of monetary or sentimental value that cannot be lost or replaced. For example:

      • Jewelry or any other valuables
      • Family heirlooms
      • Perfumes

    You are welcome to bring photos of family and friends.
