    Located on 2470 U.S. 41 North, you will find property that is rich in history dating back to the 1930s. Over time, Church Home Rehabilitation and Healthcare LLC was founded back in 1960. Chief Operating Officer Glenn Smith shares how the facility came about and what all they offer.

    “The Christ Sanctified Holy Church is a small denomination that was founded in 1892,” said Smith. “The church spread from North Carolina to all over the southeast and further west. In the 1930s, members of the church desired to buy some property so that they would have a location they could retire to as well as have a permanent location to have their camp meeting celebrations. After searching far and wide, they bought this property that we are on today.”

    At the time, the property was an existing farm, which is still managed today by Smith. It has pecan trees dating back prior to 1930, Smith said. The first camp meeting was held in 1939 and one of the longtime leaders of the church, Floyd Hagan, who was at the first meeting as a child passed away a month ago.

    “Mr. Hagan was well known in Perry because of his influence with the church and community,” said Smith. “He is truly missed.”
    Also, at the time of the purchase was a house, Smith said. In the following years, the members of the church wanted to take care of their elderly members and some of those moved into that old home place and became known as the Church Home.

    “That’s where the name comes from,” said Smith. “Over time, that evolved and in 1960, the church constructed a formal state license nursing home. In 1989, the original home was completely removed and torn down to rebuild a new one, which that structure is still in use today. The old nursing home was then completely renovated in 2012 and expanded. And once again, 2014 we expanded with our LifeSpring addition on the southern most end of the property. That addition is predominately used for our short-term rehabilitation therapies for our in-patients and out-patients.”

    Church Home Rehabilitation and Healthcare LLC is a 75-bed skilled nursing facility. They offer traditional long-term care services, as well as short-term rehabilitation.

    “If you have patients that are being discharged from the hospital and need a little more care to help gain their strength back, that is what our short-term care is for,” said Smith. “Or if you’ve had some type of surgery, hip replacement, they would need our rehabilitation services. We’ve also had patients who had strokes and needed to learn how to get back on their feet, speak and eat.”
    Smith said Church Home is not just a skilled facility, but a faith-based organization that is an outreach to the community.

    “Only 20 to 25 percent of our residents are members of the church,” said Smith. “Some think it’s just for the church members when it is actually for everyone. But it is an absolute joy for me to come and work here every day and witness the love and compassion that is offered by our team. We have a wonderful nursing and care giving team at this facility. It truly warms your heart to see the calling and gifting our staff has for taking care of our residents. It’s a blessing, it’s an honor and it’s a great body of Christ that loves the Lord and wants to share that with the community.”

    Church Home Rehabilitation and Healthcare LLC is also rated a five-star facility, which is the only one in Houston County. Smith said the rating comes from the Centers for Medicare and Medicaid Services and there are only three five-star facilities in the middle Georgia area.     Church Home opens New LifeSpring Rehabilitation Addition

    December 9, 2016—Perry, GA—Church Home Rehabilitation and Healthcare announced the opening of phase 2 in its LifeSpring section. The 6,300 square feet addition includes 10 new resident rooms dedicated to offering short-term, in-patient rehabilitation services for the community. The increase also includes additional physical therapy, dining, and common areas.

    Opening of this unit now completes a five-year effort to completely renovate the Church Home and add two wings in the newly created LifeSpring rehabilitation area. The combined Church Home and LifeSpring areas now total 75 skilled nursing beds for long-term care, short-term rehabilitation, and hospice care. In 2012, the Church Home received a complete renovation. Fourteen new rooms were later added In 2014.

    “This is a landmark day,” according to Chip Bishop, Chairman of Church Home Rehabilitation and Healthcare, LLC Board of Managers. “The completion of the renovation and additions to the Church Home is a great achievement. The Church is honored to serve the middle Georgia community by offering high quality, compassionate care through our skilled-nursing facilities.”

    Kathy Gibbs, Administrator of Church Home/LifeSpring stated, “Our focus with the new LifeSpring addition will be short-term rehabilitation for patients recovering from an injury, illness, or surgery. Many times these patients are discharged from the hospital, but still require skilled nursing care before returning to their homes. Our goal is to assist these individuals in recovering as quickly and thoroughly as possible, and then return to the community.”

    International City Builders in Warner Robins, GA served as the general contractor on the latest addition. Architectural services were provided by JMA Architecture based in Perry, GA.

    The history of Church Home traces its roots to 1939 when Christ’s Sanctified Holy Church purchased a farm in Peach and Houston counties, and began offering services to care for the elderly. Over time, the Church Home has undergone a number of improvements. Church Home has been recognized by the Center for Medicare and Medicaid Services as a Five Star Overall facility based on its quality of care. Most recently, Church Home was rated by US News & World Report as one of the Best Nursing Homes in America.

    Church Home Ranked One of Top Nursing Homes in Georgia by US News & World Report

    Perry, GA— U.S. News & World Report released on Wednesday its ranking of the best nursing homes in the nation, and three middle Georgia long term care homes were on that list. Church Home Rehabilitation and Healthcare between Perry and Fort Valley, Carlyle Place in Macon, and The Oaks in Marshallville were among 26 homes in Georgia recognized. There are 358 nursing homes certified by Georgia’s Department of Community Health.

    U.S. News evaluated more than 15,000 nursing homes nationwide to come up with slightly more than 2,000 homes included in their list of Best Nursing Homes. Millions of Americans will spend at least some time in a nursing home this year, noted Brian Kelly, editor and chief content officer of U.S. News.

    The methodology for the U.S. News “Best Nursing Home” designation relies on data from Nursing Home Compare at the federal Centers for Medicare and Medicaid Services. Factors included overall performance ratings, as well as ratings for key services over the 12 months ending in October 2016.     Church Home/LifeSpring Recognized for 10th Straight Year for Outstanding Resident/Family Satisfaction

    Perry, GA— For ten straight years Church Home/LifeSpring has been awarded the Excellence in Action Award from My Inner View/National Research Corporation.  The Excellence in Action Award recognizes the top 10% of skilled nursing, assisted living and independent living providers based on overall customer satisfaction levels.  Church Home/LifeSpring was the only facility in the Perry/Warner Robins market to receive the award in 2014 and 2015.

    The MyInner View database is the largest long term care satisfaction metrics database in the US.  In order to be eligible for the Excellence in Action Award a facility must:

    • Regularly survey residents, families of residents and staff using the My Inner View instrument
    • Receive a minimum response rate of 30%
    • Score in the top 10% of qualifying facilities on the question “What is your recommendation of this facility to others?” in terms of percentage of respondents rating the facility as “excellent”

    Church Home Rehabilitation and Healthcare, Inc. (Church Home/LifeSpring) located on Highway 41 North between Warner Robins and Perry, recently named Glenn B. Smith of Macon its Chief Operating Officer.
