    Church Home healthcare earns 2020 Silver National quality award

    Perry, Georgia – Church Home Rehabilitation and Healthcare has been recognized as a 2020 recipient of the Silver – Achievement in Quality Award for its dedication to improving the lives of residents through quality care. The distinction is the second of three progressive award levels through the National Quality Award Program, presented by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL), the leading association for long term and post-acute care. The program honors association members across the nation that have demonstrated their dedication to improving quality of care for residents and patients in long term and post-acute care.

    “It’s an honor to again be recognized for our commitment to delivering quality care,” said Kathy Gibbs, Administrator of Church Home Healthcare. “High quality of care has long been a priority at Church Home. We are supporting that care by growing our skills in quality management and continual process improvement.”

    The National Quality Award Program, which AHCA/NCAL established in 1996, is based on the core values and criteria of the Baldrige Performance Excellence Program, which is also the foundation of the metric based AHCA/NCAL Quality Initiative. The Baldrige program helps organizations in different business sectors improve and achieve performance excellence.

    The Award Program has three levels: Bronze, Silver, and Gold. At the Silver Award level, applicants continue to learn and develop effective approaches that help improve performance and health care outcomes. The Silver criteria, based on the Baldrige Excellence Framework, provide applicants with a thorough assessment of how their organization works, its effectiveness, and tools for improvement in their approaches. As a recipient of the Silver Award, Church Home may now move forward in developing approaches and achieving performance levels that meet the Gold – Excellence in Quality Award.

    “I commend Church Home for continuing on the journey to quality improvement,” said the AHCA/NCAL National Quality Award Board of Overseers Chair Alana Wolfe. “Church Home is one of only 18 facilities in Georgia receiving the Silver Award in 2020.”

    Post-Acute Care Administrator Receives ACHCA’s Eli Pick Facility Leadership Award

    Washington, DC – April 9, 2019 – The American College of Health Care Administrators (ACHCA) honored Kathy Gibbs, Administrator of Church Home Rehabilitation and Healthcare in Fort Valley, Georgia with the 2019 Eli Pick Facility Leadership Award. The leadership award was celebrated at ACHCA’s Annual Convocation and Exposition in Louisville, Kentucky on Monday, March 18, 2019. Only 7% of facilities nationwide qualified. This year, 1,129 facilities met the selection criteria. Two hundred and eighty-six administrators in 35 states were awarded the facility leadership award nationally.

    Based on the premise that facility excellence reflects leadership excellence, this award recognizes the Administrator of record who provided such leadership for the full 2018 calendar year. Eligibility for this award is based on three years of skilled nursing facility (SNF) survey data, including the Health, Fire Safety, and Complaint Surveys, as well as top quartile performance on designated Quality Measures. The criteria also include at least an 80% or greater facility occupancy and a three-year avoidance of a Special Focus Facility status. This prestigious award is made possible with the support of ABILITY® Network Inc.

    The Facility Leadership Award was introduced in 2008 by one of ACHCA’s most revered leaders, the late Eli Pick. A former executive director of the Ballard Rehabilitation Center, DesPlaines, IL for over 30 years, Eli embodied excellence as an Administrator that cared for his residents, their families, and his community. This award is presented annually in memory of Eli, a consummate member of ACHCA, dedicated to advancing professionalism and leadership in long term care.

    Founded in 1962 the American College of Health Care Administrators (ACHCA) is the only professional association devoted solely to meeting the professional needs of today’s long-term care administrator and executive. ACHCA’s mission focuses on advancing leadership excellence in long-term care through the provision of leadership education, promotion of professional advancement programs such as Fellowship and Professional Certification, and facilitation of leadership development. The Church Home LifeSpring community received recognition in The U.S. News which published its overall ratings to eligible nursing homes from High Performing to Poor.  Church Home LifeSpring received a Better than Average rating.  The rating evaluates the care of a wide range of conditions at a facility, including both long-term chronic care and shorter-term rehabilitation. The data used to evaluate these facilities come from the federal Nursing Home Compare program.

    Church Home LIfeSpring received the Excellence In Action Award for the 12th Consecutive Year for Outstanding Resident/Employee Satisfaction

    August 22, 2018—Perry, GA— The National Research Corporation (NRC)  Health awards Church Home LifeSpring the Excellence In Action Award.

    The Excellence in Action award recognizes the top 10% of providers for skilled nursing, assisted living, and independent living communities that achieve the highest level of excellence in resident and/or employee satisfaction. Customer satisfaction level is determined by survey results from residence and/or employees.   Church Home LifeSpring is honored to receive this award for the 12th consecutive year.

    The NRC Health product database is the largest proprietary satisfaction database for senior care in the U.S.  In order to be eligible for the Excellence in Action Award a facility must:

    • Regularly survey residents, families of residents and staff using the My Inner View instrument
    • Receive a minimum response rate of 30%
    • Score in the top 10% of qualifying facilities on the question “What is your recommendation of this facility to others?” in terms of percentage of respondents rating the facility as “excellent”

    Church Home Again Ranked One of Top Nursing Homes in Georgia by US News & World Report

    November 8, 2017—Perry, GA— U.S. News & World Report released its ranking of the best nursing homes in the nation, and two middle Georgia long term care homes were on that list. Church Home Rehabilitation and Healthcare between Perry and Fort Valley, and The Oaks in Marshallville were among 21 homes in Georgia recognized. There are 358 nursing homes certified by Georgia’s Department of Community Health.

    U.S. News evaluated more than 15,000 nursing homes nationwide to come up with slightly more than 2,300 homes included in their list of Best Nursing Homes. “Millions of Americans will spend at least some time in a nursing home this year,” noted Brian Kelly, editor and chief content officer of U.S. News.

    The methodology for the U.S. News “Best Nursing Home” designation relies on data from Nursing Home Compare at the federal Centers for Medicare and Medicaid Services. Factors included overall performance ratings, as well as ratings for key services over the 10 months ending in August 2017.
