FAQ

Our campus provides a welcoming environment where you will feel comfortable, safe and right at home! Our residence is well maintained, and our guests often comment on how clean the facility is. The grounds are beautifully landscaped for your enjoyment, and our walking paths create an idyllic setting. Our common areas invite families to linger for long chats and to create new memories.

We have over 100 dedicated care members on our campus – from Dietary, Environmental Services, Nurses, Certified Nursing Assistants, Therapists, Activities and Administration – who are proud to be a member of the LifeSpring Team!

We maintain an agency-free facility, which gives consistent care to our residents. We also retain excellent staff-to-resident ratios. Certified nursing care, dietary and housekeeping services are provided twenty-four hours a day, seven days a week.

All laundry is done on-site with a 24-hour turn around for personal clothing. Responsible parties are also welcome to take care of the resident’s laundry, as well. Please label any clothing that comes in the facility – even if family is responsible for washing. Accidents may happen and we want to make sure the clothing is returned to the correct individual.

All nursing home residents have rights and protections under the law. The Resident Rights are displayed in the hallways of Church Home and Jacob’s House and a copy is given to the responsible party of all residents upon admission.

Our residents review Residents Rights at monthly Resident Meetings and we strive to always treat everyone with respect and dignity.

We follow guidelines and recommendations from numerous organizations such as Centers for Disease Control and Prevention (CDC), Departmen of Public Health, Centers for Medicare and Medicaid Services (CMS) when outbreaks occur. We have many measures in place to protect our residents and staff from illnesses and to minimize risk of exposure when caring for those who are ill.

We have stringent infection prevention and control measures, frequent hand washing and cleaning of our home and extra precautions when caring for residents. We closely monitor our residents and staff for illness and instruct staff to not report to work when they are sick. At times, we adjust our visitor guidelines to ensure we maintain a safe environment.

We pride ourselves in offering health education to the public and make vaccines available for our residents and staff to protect themselves from COVID, Flu and other illnesses.

LifeSpring is one of very few, mission-driven nonprofit nursing homes operating in Georgia. As a 501©(3) organization, all donations are tax deductible. We believe our nonprofit status is a fundamental difference in our approach to resident care. Donations go directly into providing excellent experience and an improved quality of life for our residents, not into stockholders accounts. For over 80 years, LifeSpring’s management has focused on this mission. We continue to uphold a long-standing tradition of serving Seniors with God’s love.

Approximately each year, the Georgia Department of Community Health sends an Inspection Team unannounced to nursing homes throughout the state for an annual survey of their operations and facilities. Any issues found in this survey are subject to correction within 90 days.

A current survey book is kept in the front lobby of Jacob’s House, Church Home and Grace Garden for your review at any time. Surveys are also accessible on the CMS Nursing Home Compare Website.

LifeSpring partners with local hospice providers to add an additional layer of care for those who have a terminal diagnosis and desire hospice services. Hospice doctors and nurses work together with our Care Team to oversee and manage end-of-life care.

It is important that residents maintain relationships with their family and friends. We desire to make it easy for residents to spend visiting others. We include guests in our activities, meals, celebrations and holidays. Our facility does not put a timeframe on visitation; however, we do not allow overnight guests. A door code will be provided upon arrival.

Because this is our residents’ home, we do not maintain set visiting hours; however, we ask guests to respect the privacy and rest of our residents and ask that visits occur between the hours of 9 am and 6 pm.

At LifeSpring, your safety and health are our top priorities. That is why we have installed the latest state-of the-art security system with controlled access, security cameras throughout the facility, smoke and fire sprinkler system and locked entrances. Additionally, our Safety Committee meets quarterly to review, develop and implement safety procedures for our staff and residents. We also facilitate frequent safety training and drills to ensure we are prepared to respond in the event of an emergency.


Comfortable everyday clothing, non-skid shoes, personal hygiene products (basic products are provided by facility), only safety or electric shavers, copy of Medicare and all other insurance cards, social security card, photo ID, and copy of any advanced directives or living wills, if applicable.

Admission and financial agreements must be signed within 24 hours of admission. Residents and responsible parties will meet with the admissions team to explain the admissions agreement and answer your questions. Documents are sent electronically via Docu-sign. Access code to sign documents is: LSCGA.

New residents will be seen by our Medical Director within 48 hours of admission. Residents will not see the physician daily. The Nurse Practitioner will be in the facility to visit you, if needed twice a week. The Interdisciplinary Team, which also consists of the Medical Director, meets weekly to discuss all residents who are in Jacob’s House receiving rehabilitation services. Our nursing team can also reach out to the Medical Director if there is an issue.

Please do not bring any medications into the facility. Medications from our pharmacy, which have been reviewed by the Medical Director and Pharmacist, are the only medications to be administered for the safety of the residents.

In the case that you must go out to the hospital, you will immediately receive information regarding bed hold. Please understand this is a requirement of Medicare and must be given to the resident or responsible party at the time of transfer to the hospital. The health and safety of our residents is our first priority; however, due to the requirements of Medicare, we will be reaching out to the responsible party regarding bed hold. We apologize in advance if the timing of this question seems insensitive – it is purely regulatory.

Per Medicare guidelines, resident’s that are admitted to Jacob’s House for in-patient rehab services are not allowed to go out for overnight visits unless being admitted to the hospital. Long-term care residents are welcome to go out with family members as their health and medical supervision permit.

Our cable company is Sparklight, and we are subject to their channel listing. Unfortunately, the listing cannot be customized by room. A channel listing will be provided upon admission. Please feel free to use a laptop or iPad device if you have certain programs that you wish to view. Our guest Wi-Fi is free to use. You may also utilize streaming services using your own subscription services.

We are a non-smoking campus. No smoking or vaping is permitted anywhere on the premises.

Please do not bring disposable wipes or flushable wipes to the facility. Although advertised as such, no wipe products are appropriate to be flushed into any sewerage system. While we understand the desire to use wipes, unfortunately they cannot be accommodated in our sewage system. We will incur fines from the city of Perry if they are found in our system.
