    Happy new year! The new year is a time to reaffirm and enjoy the start of a bright and promising new year. We hope you're looking forward to having new adventures and creating new memories with your friends and family in 2019.   Many will also make it a time to consider resolutions for the new year.

    In planning your year, we encourage you to place your health and wellness need at the forefront.   At Church Home LifeSprings we understand the unique needs of seniors and carefully designed care programs.  Our programs include encouragement and connection as well as personalized rehabilitation services to meet your short and long-term needs.     Person-Centered Care

    Our commitment to person-centered is to evaluate patient individual and unique needs and wishes for recovery. Each care plan is customized to meet those needs. This can include the amount and type of professional care the person receives and personal preferences.  We build relationships with our patients and residents and encourage independence to the level of their ability and desire.

    Staff members receive ongoing training to ensure they are providing the highest level of care to our residents. Families are encouraged to be a part of the process, and we are committed to keeping everyone engaged and informed as we listen to and address any concerns you might have.  

    Short-Term Rehabilitation

    After a hospital stay, surgery, or illness, a short-term rehabilitation program may be required to get back on their feet.   Our focus is improving skills and getting our residents back home as soon as reasonably possible.  Our skilled team of therapist employ the latest technology and committed to maximizing recovery outcomes and minimizing recovery times.

    Long-Term Rehabilitation
